If this is an application for a new business or for a new owner of an existing business, complete the application in its entirety, execute the application, complete the appropriate affidavits, attach all applicable documentation described in the application check-list, and submit a non-refundable application fee of $500.00 with the completed application. If the application is approved, additional licensing fees shall apply.
If this is an application for renewal of an existing license, complete the application in its entirety, execute the application, complete the appropriate affidavits, attach all applicable documentation described in the application check-list, and submit the annual renewal fee with the completed application.
